PHYSICAL THERAPY APPOINTMENTS

Your first visit will focus on gathering information, evaluation of your problem, and determining a treatment plan. Some treatment and/or exercises are usually initiated. Followup visits focus on treatment of your problem. A visit frequency of one to two visits/week for four to six weeks is usually preferred. Further treatment will be determined by outcomes and what is appropriate for your condition. Some problems may only require a few visits, others will require more. Treatment time will generally be 40-55 minutes (per therapist's discretion). Having intake forms ready prior to your first appointment is preferred. Loose clothing will make it easier for hands-on treatment. PLEASE AVOID USING SYNTHETIC SCENTS (i.e. perfume or cologne) AT THE TIME OF YOUR APPOINTMENTS. Cancellations must be made 24 hours in advance to avoid a same day cancellation or "no show" fee.

Jeff State Physical Therapy accepts most major insurance carriers and is in network with most providers. As a courtesy we will bill your medical insurance and will assist you with coverage information. Please note, we cannot guarantee insurance information and recommend that you contact your insurance directly to determine benefits. While we are happy to help, it is the patient's responsibility to be informed of insurance benefits. We also offer competitive cash pay rates - please enquire for pricing.
